Basics
- Must be 18 years of age
- Must have Class D driver license for at least 1 year
- Pass vision test
- Pass color blindness test
Documents
- Need a department of Transportation medical certificate
- Complete the Utah self-certification form
- Proof of identity
- Provide proof of legal presence
- Provide proof of Utah residency
- Provide your social security number
- Complete training provided by an authorized provider
